Survey Monkey Millions  of people use SurveyMonkey for everything from customer satisfaction and employee performance reviews, to course evaluations and research of all types. (Survey monkey) The tool itself is very user friendly, it has custom set  questions that you can use for your survey or you can  create your own.  It allows for multiple choice, star rated, open and closed ended questions.  Libraries and researchers can really benefit from this tool.
